What the form does
The quote form asks for name, phone, email, city or ZIP, service type, and notes. It does not ask for a street address. Until a form endpoint is configured, submissions stay in the browser and show a preview success state.
When connected
Before launch, the operator should document the chosen form processor, retention, access, deletion requests, and any analytics or cookies. Do not connect a production endpoint without reviewing that policy.
